With Google Earth, I plotted the three NOTAM boxes. They clearly define
a great circle path to the southwest that includes the SLC-3 launch
site. The azimuth is 222.3 degrees. After extending that line through
the equator, I measured the heading from the equator crossing as 213.5
degrees. That's an inclination of 123.5 degrees, reasonably matching
    Ted's calculations.

So what hazard do each of the boxes represent? There were no SRBs on
this flight, so there would be no impact areas for them. The closest box
is probably for an early launch abort near the pad. The second is
probably the payload fairing impact area, and the third would be the
    first stage impact area.
