Based on the tle for Object A, and typical trajectory used by the rocket, it to me looks like the
upperstage failed after the 3rd burn. So 4th burn looks like the that failed.

I wonder of the APT was released, after the 3rd burn, and before the 4th burn.

Now to wait and see what ILS says, when they release more details.
