Many astronomy references use altitude.  Most space & satellite references 
dating back to the days of Sputnik use elevation for angular distance 
above horizon, and altitude/height for distance above earth surface.
 
So I go along with this and use elevation for angle to look up, and 
altitude for height in miles/km.

I had discussed this with Mike McCants a few years ago and he graciously 
changed al to el in his great QUICKSAT program.
