The spiral in the sky phenomenon is quite striking. I've seen many photos and videos of it from I think Finland? Norway? I forget. Like you guessed it is basically a serious failure where the thrust isn't axial - isn't symmetric. For example if only one engine was working or if half the exhaust cone ripped off. The result is a rocket spinning in a fast circle with exhaust spreading out in a spiral at the same time as the object orbits (or coasts) overhead.
