    PSLV final stage disintegration ?

    PSLV DEB 682 x 617 km
    1 27165U 01049DM 02014.46661551 .00036947 00000-0 55384-2 0 16
    2 27165 98.0345 91.9346 0046416 0.2637 359.8476 14.73746363 3282

This is the latest PSLV fragment catalogued by OIG and they are already at
DM. It seems obvious that 'something' has happened.
