The best I know of is Desmond King-Hele, "Observing Earth Satellites". 
2nd edition is 1983, still well before the era of PC tools and
net-distributed elsets.  

Probably out of print, but worth the effort if you can find a copy for
do-it-yourself-on-paper predictions and such.
